The written statement form used for Unauthorized/Improper ACH activity is changing, effective March 19, 2010.

The  phrase "under penalty of perjury" is being deleted.  The intent of this change is to avoid triggering any state's requirement that such a statement be notarized.

 

National Bank Products forms STUIACH-WS and UIACH-WS will be revised.  We will have these ready for shipping in February 2010.  The two forms used in states that required a notary section will be eliminated (STUIACH-AN and UIACH-AN). 

 

This is the one change in the months ahead that makes good sense.
