Standard Flood Hazard Determination Form - Friday, January 30, 2009

FEMA has updated the Standard Flood Hazard Determination Form (SFHDF). The prior version expired on October 31, 2008 has been replaced with a similar form that expires December 31, 2011. The revised form includes a new Office of Management and Budget (OMB) control number. The form's format and content have not changed. The updated form must be used beginning June 16, 2009.
