The IRS has posted a revised W-9 form on their website.  The form has a revision date of September, 2007.  The changes to the form include a new checkbox on page one for Limited Liability Companies along with a space to enter a tax classification for the LLC.  There are also changes to the instructions on page one and page two has changes to the LLCs instructions.  A new section titled Secure Your Tax Records from Identity Theft was added to page four.
